Request a Re-activation for an Existing Supplier
Some suppliers might already exist in 91黑料网's Financial Information System (Banner), but may have been terminated due to inactivity during the previous 24 months. Please use the Banner form FTIIDEN to confirm if a supplier ID number has been created. If one exists, please go to MMP and fill in the Reactivation/Change Request form.聽Once vetted, the supplier will be re-activated.聽
