Student Clinical Placement Information Form
Students must keep their information up to date in Minerva and in the Student Clinical Placement Information Form. For clinical placement planning and coordination, students must enter in the Student Clinical Placement Information Form the address where they will be residing during the clinical placement, their contact information, any potential conflicts of interest and the required signed consents.
